The Liver System in Traditional Chinese Medicine

The Liver in TCM is known as the "General" — responsible for the smooth flow of Qi throughout the entire body. This function, called "coursing and discharging" (Shu Xie), affects digestion, emotional regulation, menstruation, and the flow of bile. When Liver Qi flows smoothly, emotions remain balanced, digestion operates well, and menstrual cycles are regular. When Liver Qi stagnates, frustration, irritability, sighing, distension in the ribs, and menstrual irregularities commonly follow.

The Liver stores Blood, particularly during rest and sleep, and releases it to nourish the tendons and eyes during activity. This is why the Liver opens to the eyes — blurry vision, floaters, and dry eyes can all indicate Liver Blood deficiency. The Liver also governs the tendons and is reflected in the nails. Brittle, ridged, or pale nails may suggest insufficient Liver Blood nourishment.

Emotionally, the Liver is associated with anger and its related states: frustration, resentment, and feeling constrained. Chronic emotional suppression or stress frequently leads to Liver Qi stagnation, one of the most common patterns seen in modern clinical practice. Acupuncture point LR-3 (Taichong, "Great Rushing") is one of the most frequently used points for promoting the smooth flow of Liver Qi.

Frequently Asked Questions

What does the Liver do in Chinese medicine?

The TCM Liver ensures the smooth flow of Qi throughout the body, stores Blood, governs the tendons, opens to the eyes, and is reflected in the nails. It plays a central role in emotional regulation, digestive harmony, and menstrual health. Liver Qi stagnation is one of the most frequently diagnosed patterns in clinical practice.

How does stress affect the Liver in TCM?

Chronic stress, frustration, and emotional suppression cause Liver Qi to stagnate. This can produce symptoms including rib-side distension, sighing, irritability, tension headaches, digestive upset, and menstrual irregularities. Prolonged stagnation may generate heat, further complicating the pattern.
